This role offers an exciting opportunity to work in a dynamic environment with a dedicated team.Role Overview:

• Conduct comprehensive face-to-face assessments of patients to determine their needs.
• Perform regular ward visits to engage with patients, families, and the healthcare team.
• Facilitate timely and safe discharges from the hospital.
• Work within a hybrid model, combining on-site presence and remote work.
• Collaborate closely with multidisciplinary teams, attending meetings and contributing to care planning and risk assessments.
• Maintain accurate records and prepare detailed reports.
• Ensure compliance with relevant legislation and policies.
• Utilize strong interpersonal skills, empathy, and an understanding of the challenges faced by individuals in a hospital setting.

Essential Requirements:

• Social Work Qualification – Degree or equivalent
• At least 2 years’ experience as a social worker within children’s services
• Social Work England Registration
• Driving license and access to own vehicle
