The Health Equity Programme team plays a central role ensuring that reducing inequalities in outcomes, access and experience is at the heart of everything the integrated care system across North West London does. The programme team does so through three areas:Working with and through North West London’s health and care commissioners and providers to identify and tackle inequalities in access, experience and outcomes, using national guidance (such as the Core20Plus5 framework for adults and children) and local needs and experience to focus the team’s effort.Working with partners across North West London to address the wider determinants of health. This includes:Supporting to people to live healthily;Using strengths of North West London’s Anchor institutions to support and strengthen our communities, including contributing to wider economic and socialdevelopment, with a focus on our most deprived communities.Embedding a Population Health Management (PHM) approach to create a robust, methodologically sound, approach to reducing inequalities. This includes building capability and capacity in the skills needed to deliver PHM.Main duties of the jobWorking with internal partners across North West London and external to address the wider determinants of health. This includes:Supporting to people to live healthily;Using strengths of North West London’s Anchor institutions to support and strengthen our communities, including contributing to wider economic and social development, with a focus on our most deprived communities.To lead on a range of key projects, working with the Head of Wider Determinants to ensure that there is a co-produced approach to continuously identifying barriers and inequalities and overcoming these.To work with a ICS partners in relation to the Anchor portfolio. This will involve leading on Anchor Steering Groups and Community of Practice Forums to facilitate sharing of best practice and facilitate close working with anchor organisations.To oversee delivery and monitor impact of NWL Anchor Charter and future developments of the CharterTo oversee and monitor impact of the broad range of volunteering programmes to support Core20+5 groups into volunteering and employment in the NHS.To support the implementation of the digital inclusion project recommendations so that it is embedding into everyday working practices.To act as the equity Relationship Manager for at least one NWL ICB Programme and Borough team.About usNHS North West London Integrated Care Board (NHS NWL ICB) is a statutory body responsible for planning and allocating health and care resources to improve the lives of people in eight boroughs across North West London: Brent, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow, Royal Borough of Kensington and Chelsea and Westminster.The local population is large and diverse, with a wide range of health and care needs that are evolving all the time.
