What you will do:

• Provide strength-based support, building on existing skills and resources.
• Collaborate with specialist care providers to deliver integrated services focused on recovery and independence.
• Apply comprehensive knowledge of relevant legislation, such as the Care Act and Mental Capacity Act.
• Conduct thorough social care needs assessments, Human Rights assessments, and mental capacity assessments.
• Work directly with clients and families, offering guidance and intervention to support their wellbeing and autonomy.
• Manage risks and problem solve complex cases while adhering to legal frameworks.
• Maintain positive relationships with key stakeholders and promote quality service delivery.
• Engage in face-to-face work as required, with flexible opportunities for working from home.
