Adult SW will receive £1500 pro rata, and AMHP SW will receive £2500 pro rata in their first salary paymentJob termFull TimeAppointment typePermanentHours37 hours per weekClosing Date28 April 2024An exciting opportunity has arisen for an enthusiastic and motivated Social Worker to join the Erewash Short Term Assessment and Reablement Team.The Erewash Short Term Assessment and Reablement Team provides a multi-disciplinary approach to assess, support, and enable adults to maximise their independence and to assess for any potential long term care needs. We are committed to working in a person centred and strengths-based way, recognising people’s assets and strengths and enabling them to identify and reach their goals. The team are community based but will also support people leaving residential and nursing establishments and will include facilitating safe and timely discharges from some hospital settings.We are continuing to offer a hybrid approach to working, and our team is currently working from home and from the office based on the needs of the service.The successful applicant will be able to demonstrate the necessary skills, knowledge, and experience to undertake this role in order to provide a safe, accountable, cost effective and efficient service to our clients which helps them to achieve their outcomes. You will need to demonstrate your ability to complete social care assessments and support plans in line with the Care Act 2014 and the Mental Capacity Act 2005. We expect that you will be confident in making decisions and managing a busy caseload with good, clear communication and partnership working. You will be part of a duty rota and will undertake Safeguarding enquiries in line with the Care Act 2014. The post is for a 5 from 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis.
